School Year 20-21 Program Student Registration

Parents of NYC students who attend a Seventh-day Adventist School can enroll them in this FREE program for students in grades K-12 created to supplement each student’s learning experience.

Due to the unique circumstances of this school year, the program will take place in small groups through in-person or remote learning. Sessions are designed to include a holistic experience for students composed of targeted math and literacy instruction as well as STEM and SEL activities to encourage personal growth in a collaborative classroom.

By submitting this form, I acknowledge and agree to the following:
(1) I am providing consent for my child to participate in the program.  
(2) I agree to receive communication via email, text, and phone regarding updates to this program.  
(3) I permit my child’s Catapult Learning teacher to access their assessment scores for the purpose of instructional quality.  
(4) I consent to my child’s image (photo or video) being used for promotional materials.
